- Presenting issue
- Lateral elbow pain for 14 months, dominant arm. Failed 12 weeks of PT and two corticosteroid injections; symptoms returned within 6 weeks of each.
- Approach
- Single ultrasound-guided leukocyte-poor PRP injection with needle fenestration, then a progressive eccentric loading program.
- Outcome
- Pain 7/10 → 3/10 at week 8. Grip strength improving; back to full work duties.
Author asks: Second injection at week 12, or keep loading and reassess?
I'd hold. When loading compliance is good the curve keeps bending through months 4–6. I reassess at 16 weeks before offering a second injection.
