Interesting PRP case from this week. Patient with chronic lateral epicondylitis after failed PT and two steroid injections. Went with a single ultrasound-guided LP-PRP injection plus a progressive eccentric loading program — first time I've seen grip strength move this early. Curious how others time a second injection, or whether you hold.

Presenting issue
Lateral elbow pain for 14 months, dominant arm. Failed 12 weeks of PT and two corticosteroid injections; symptoms returned within 6 weeks of each.
Approach
Single ultrasound-guided leukocyte-poor PRP injection with needle fenestration, then a progressive eccentric loading program.
Outcome Improving
Pain 7/10 → 3/10 at week 8. Grip strength improving; back to full work duties.

Second injection at week 12, or keep loading and reassess?

I'd hold. When loading compliance is good the curve keeps bending through months 4–6. I reassess at 16 weeks before offering a second injection.
